Performance measurement plans provide a framework for implementation. They describe how you will measure performance and what information will be collected. Performance measurement plans consist of information in the performance measurement PFI work plans, including the service activity, results, indicators, targets, and instruments to be used. They also include specific information on when data will be collected, how often, and by whom, as well as who will be responsible for analyzing data and reporting the results.
