Health Screening Release
A health screening release form can be useful for programs that want to host a health fair. This sample was submitted by TenneSenior Service Corps, a project of the Coalition for Healthy Aging.
Media Release Form
Sample media release form authorizing permission for an individual's words and images to be used for promotional and informational purposes, from Community Youth Services.
Photo Release (ID 245)
This simple photo release form includes the Senior Corps logo, but could be adapted to any program. Grants rights to both web and print use in perpetuity.
