Borrowing privileges are available ONLY to programs or members funded by the Corporation for National and Community Service.

By completing this loan agreement you claim responsibility for the following:

* Notify the library if materials are damaged on arrival
* Return materials by due date or contact library for extension
* Use adequate protective packaging for material returns
* Pay return shipping costs (via Fed Ex, UPS, or USPS)
* Pay replacement costs if materials are damaged or lost
